چکیده

BACKGROUND AND PURPOSE Arterial spin labeling (ASL) is a perfusion magnetic resonance imaging (MRI) technique that does not require contrast administration and thus may be more practical in hyperacute stroke than susceptibility-weighted bolus-tracking perfusion-weighted imaging (PWI). However, a threshold for ASL measurement of the ischemic penumbra needs to be determined. METHODS A total of 58 patients with acute hemispheric ischemic stroke were imaged within 6 hours of symptom onset with MRI including ASL, diffusion-weighted MRI (DWI), and PWI, after perfusion computed tomography (CTP). Patients had repeat MRI at 24 hours. On repeat imaging, 32 patients did not reperfuse and were used to determine the penumbra threshold. A receiver-operating curve and a volumetric analysis were undertaken to identify the ASL-cerebral blood flow (CBF) threshold for the acute penumbra compared with the 24-hour DWI lesion in patients without reperfusion and with the acute PWI and CTP-Tmax thresholds. RESULTS An ASL-CBF threshold of 40% showed the highest area under the curve (AUC) for detection of the acute penumbra, defined by 24-hour DWI in nonreperfused patients (AUC 0.76, 95% confidence interval, 0.63-0.85), and was also accurate compared with the acute PWI-Tmax+6 seconds penumbral threshold (AUC 0.79, 95% confidence interval, 73-84) and acute CTP-Tmax 5.5 seconds penumbral threshold (AUC 0.77, 95% confidence interval, 72-84). Using a perfusion-to-diffusion mismatch ratio of 1.8:1, an ASL-CBF 40% mismatch compared with PWI-Tmax+ 6 seconds-DWI and CTP mismatch showed good sensitivity (0.81) and specificity (0.71). CONCLUSIONS ASL-DWI mismatch shows potential to identify salvageable tissue in hyperacute stroke.

Quantitative Imaging of Perhsion Using a Single Subtraction (QUIPSS and QUIPSS

In the pulsed arterial spin labeling (ASL) techniques EPISTAR, PICORE, and FAIR, subtraction of two images in which inflowing blood is first tagged and then not tagged yields a qualitative map of perfusion. An important reason this map is not quantitative is that there is a spatially varying delay in the transit of blood from the tagging region to the imaging slice that cannot be measured from ...

Perfusion information obtained by dynamic contrast-enhanced phase-shift MRI: Comparison with model-free ASL

Introduction The relationship between the phase of the MRI signal and the concentration of contrast agent enables registration of arterial input functions (AIFs) in bolus-tracking perfusion MRI, with potential advantages such as favourable SNR and good linearity with concentration [1]. Already in 1992, the phase shift in tissue as a function of time after a bolus injection of contrast agent was...

Per-subject characterization of bolus width in pulsed arterial spin labeling using bolus turbo sampling.

Quantification of cerebral blood flow using QUIPSSII pulsed arterial spin labeling requires that the QUIPSS saturation delay TI1 is shorter than the natural temporal bolus width. Yet the duration of the bolus of tagged spins entering the region of interest varies during vasoactive stimuli such as gaseous challenges or across subjects due to differences in blood velocity or vessel geometry. A ne...

Novelties in radiology – Arterial spin labeling , the gadolinium - free MR perfusion

Arterial spin labeling (ASL) is a recently developed magnetic resonance (MR) technique that assesses cerebral blood flow (CBF). This method has been mainly used for investigative purposes with very few centers in the world performing it on a routine clinical basis. ASL has already been validated and proven to be useful in the assessment of a growing number of diseases and conditions. As with an...
